How does Avalon Physician Services work?

If you would like to schedule an appointment with one of our medical providers, call us at 1-888-882-3783 or submit an appointment request here. Please have your insurance coverage information readily available.

Once you have scheduled your initial appointment, we ask that you complete the relevant new patient forms to allow providers more time to focus on your medical care. Please submit these forms within 2 – 3 days of scheduling an appointment. If you have any questions please give us a call at 1-888-882-3783.

Our medical providers will then visit your home at the scheduled day and time. Please make sure to prepare all your medications, vitamins, home remedies, and prescriptions; copies of medical logs and test results; any hospital discharge papers and patient summaries. During the initial visit, our team will perform a comprehensive physical examination and include any necessary diagnostic and medical testing. They may make recommendations for patients to visit a specialist or for follow-up appointments.

Our medical providers aim to ensure that the patient is comfortable and receives the highest level medical care in the comfort of their own home.

Are there any fees or extra costs for a House Call?

Never! There are zero extra or upfront costs for a house call.

Who makes the house call?

Our team consists of experienced multilingual doctors, nurse practitioners, and physician assistants. All of our nurse practitioners and physician assistants are board certified. They are specially trained and certified to provide comprehensive medical care, coordinated care and diagnosis.

What area does Avalon Physician Services accommodate?

We happily serve the metro Detroit and surrounding areas.

Are there any copayments/deductible fees for the visit?

Upon scheduling your first appointment, Avalon Physician Services will inform you of any deductible and/or copayment which may apply.

What if I have a life threatening emergency?

Please call 911 with any immediate life threatening conditions such as chest pain, shortness of breath or head injuries with loss of consciousness. It is also important to notify Avalon Physician Services at 1-888-882-3783 as soon as possible with details about all emergency room visits and/or hospital admissions.

 

What if I need a medication refill/prescription?

Please call us at 1-888-882-3783  about a week before you run out of your medications. Always report any medication side effects.
