You should have received an email from us that outlines the steps to activate your Patient Portal account. As part of the activation process you will need to call the Avalon Office at (248) 277-3110 to request your temporary password.
If you have not received an email from us, please call the office at (248) 277-3110 and you will receive help and information to activate your Patient Portal account.
Once you login with your temporary password, follow the prompts on the Patient Portal screen to set your security questions and to change your temporary password.
After you change your temporary password, you should be all set! We recommend you bookmark the Avalon Patient Portal in your browser for easy future access.
