  • Am I able to get x-rays done in my home?

    Yes! We offer the same labs and tests you can get at your doctor's office, all in the convenience of your home - available in as little as one day!

  • What is CCM?

    Chronic Care Management (CCM) is a program that our office utilizes to help service our patients in providing them with care to help improve and educate on the chronic ailments a patient may have. In order to qualify for CCM, a patient must have 2 or more chronic conditions. Such chronic conditions include diabetes, hypertension, and asthma. This program helps patients by offering plans to better help control and/or manage chronic conditions. When helping manage a chronic condition, patients become more aware and able to recognize when something is not right and to get treatment sooner.   • What is RPM?

    Remote Patient Monitoring (RPM) is a program offered by Avalon that allows providers and nurses to work together with patients and families in monitoring chronic conditions such as hypertension, and asthma, in the comfort of their own home with a cellular Bluetooth device. This device alerts the patient and provider of abnormal results in real time. When providers and nurses are aware of a real time event the patient has a more immediate intervention than having to wait until an appointment.
